DURING ENROLLMENT Upload the scanned or digital copy (*jpeg, *jpg, *png or *pdf format) of the following requirements
FRESHMEN
    1. Back to the back copy of the 2nd or 3rd Quarter of Grade 12 Report Card (DepEd F-138).
    2. Clear copy of the applicant’s unedited 2×2 picture (white background, collared shirt) for the temporary e-ID.
TRANSFEREE
  1. True Copy of Grades (TCG)*
  2. Honorable Dismissal/Transfer Credentials*
  3. Certificate of Good Moral Character*
  4. Clear copy of the applicant’s raw 2×2 picture (white background, collared shirt) for the temporary ID

The important thing to note is that If any of the crucial requirements are missing, your application will be put on hold.

FOR INTERNATIONAL STUDENTS

FROM ABROAD (Freshmen and Transfer Student)
  1. Copy of the Official Transcript of Record/Scholastic Records by the Philippine Embassy or Consulate in the student’s country of origin or legal residence.
  2. Photocopy of passport data page containing the student’s full name, date and place of birth, passport number, and photo.
  3. 2×2 ID picture with white background
FROM THE PHILIPPINES
FRESHMEN
  1. Back-to-back copies of the 2nd or 3rd Quarter of Grade 12 Report Card/DepEd F-138.
  2. Clear copy of the applicant’s raw 2×2 picture (white background, collared shirt) for the temporary e-ID.
TRANSFEREE
  1. Photocopy of passport data page containing the student’s full name, date and place of birth, passport number, and photo.
  2. 2×2 ID picture with white background.
  3. Honorable Dismissal/Transfer Credentials

AFTER ENROLLMENT

On or before August 17, 2021, deliver all original hard copies of the following required documents to the Admissions and Student Recruitment Office (ASRO) at Mandaue City College in Mandaue City, Cebu in the Cardinal Santos Building at 900 San Marcelino Street, Ermita, Manila. This will serve as a permanent record for you:

FRESHMEN
  • Original copy of Grade 12 report card (Form 138) with the applicant’s eligibility for admission to college duly signed by the school principal.
  • Original copy of the Certificate of Good Moral Character (with the school seal)
  • Original copy of PSA Birth Certificate
  • 2pcs 2×2 ID picture
TRANSFEREE
  • Original True Copy of Grades (TCG)

All documents must be properly authenticated/apostille-ed by the Philippine Embassy or Consulate in the nation where the institution is located in order for those pupils to study abroad.
